Required Tdap Shot for 8th Graders

NEW THIS YEAR: The State of Missouri is requiring all incoming 8’th graders to have a new shot called Tdap (Tetanus with Pertussis). BEFORE SCHOOL REGISTRATION DATE, you must provide the school nurse with a physician record including date of the Tdap vaccine. If your child has received a Td (tetanus shot) within the last 2 years then they are exempt from this rule. Record of Td must also be on file with the school nurse. This is a requirement of the Missouri Department of Health. Per the state immunization laws, we are not allowed to let students attend school until they have had submitted proof of immunization.

Each year, every returning LPS student must have a Health Update Form completed by their parents. This form also gives consent for the nurse to administer Tylenol. If your child will be taking medication other than Tylenol at school, please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy please complete an Action Plan and obtain a physician signature.

Health Screenings

There are several Health Screenings completed for the student’s here at South Valley Junior High School. The purpose of these screening is to identify potential health problems in the students.  If a potential problem is identified then a referral letter will be mail to the student’s parents.  If there is no problem indicated and the student is within normal limits for that particular screening then a letter is sent home via the student that the screening was completed and the results for within normal range.  Parents, Teachers, and Counselors can request that a screening be completed for a student at any time. 

 At South Valley Junior High School the screenings that are completed are as follows:

  • 8th Grade
    • Scoliosis Screening - Completed in the spring months
  • Freshman
    • Vision Screening - Completed in the Fall
    • Blood Pressure - Checks may be completed during Health class as time allows.
    • Scoliosis Screening - Only for students that were at risk during 8th Grade.
